Before your appointment

A few simple ways to prepare.

Follow any instructions the clinic provides when your appointment is scheduled. If you are unsure about what is needed, contact the team before your visit.

What you may want available

  • Identification and current insurance information, if applicable
  • A current medication list, if relevant to your care
  • Relevant provider instructions or records you already have available
  • Questions you would like to discuss with your therapist
  • Any appointment or paperwork instructions provided directly by the clinic

Please use the clinic's approved process for any requested health or insurance information. Do not send sensitive information through an ordinary website contact form.

During your evaluation

Time to listen, learn, and understand your goals.

The exact evaluation depends on your individual needs. It may include the following general elements.

  1. 01

    A conversation about your goals

Your therapist may ask what you hope to improve and which everyday activities are most important to you.

  2. 02

    A review of relevant information

You may discuss the reason for your visit and information that helps your therapist understand your current needs.

  3. 03

    Movement and function assessment

As appropriate, your therapist may observe movement and consider mobility, strength, balance, or other functional activities.

  4. 04

    Education and recommendations

Your therapist can explain findings, answer questions, and discuss possible next steps for care.

What to wear

Choose comfort and easy movement.

Wear comfortable clothing that allows you to move freely. Your therapist may need to observe movement or access the area being evaluated, so flexible, practical clothing and supportive footwear may be helpful.

After the evaluation

A plan for what comes next.

Your therapist may discuss findings, answer questions, and recommend a care approach based on the evaluation and your goals. The clinic can explain scheduling and any next-step instructions that apply to you.
